- Birth Certificate
- Baptismal Certificate if child is baptized
- Immunization Record
- Report Card, most recent academic record if entering Kindergarten
- Sacred Heart Placement Test
- Registration Fee of $325 (Money Order or Check preferred)

Parishioner Rate (Grades K-8): |
$2,950.00 per year or 10 monthly payments of $295.00 for 1 child |
$4,550.00 per year or 10 monthly payments of $455.00 for 2 children |
$6,050.00 per year or 10 monthly payments of $605.00 for 3 children |
Non-Parishioner Rate (Grades K-8): |
$3,250.00 per year or 10 monthly payments of $325.00 for 1 child |
$5,300.00 per year or 10 monthly payments of $530.00 for 2 children |
$6,900.00 per year or 10 monthly payments of $690.00 for 3 children |
