School File Documents

Reminder: you can start submitting your child's school documents for their file for the upcoming school year. Required documents are:

• Birth certificate
• Social Security card
• Immunization record
• Recent physical
• Report card (grades 1 and up)
• IEP (if applicable)
• Sacrament documents (if applicable)
